As an independent private practice, we’ve partnered with Sharp Community Medical Group to use Sharp's Epic MyChart Portal. This enables you to access all your records across all organizations that use Epic — including Sharp, Scripps, UC San Diego, and others.
What you can do in MyChart
- View test & lab results
- Request prescription refills
- Message your care team
- Schedule & manage appointments
- Review visit summaries
- Update your health information

MyChart is a secure portal hosted by Sharp Community Medical Group on Epic’s MyChart platform. Logging in will open mychart.et1275.epichosted.com in a new tab.
Need Help?
First time signing up
You’ll need an activation code, which is included on your after-visit summary or printed on your billing statement. If you can’t find one, give our office a call and we can help you get started.
Trouble logging in
Username and password issues are handled directly by Sharp’s MyChart support — they have a dedicated help line, accessible from the login page once you click “Forgot username” or “Forgot password.”
Questions about your care
For anything related to your medical care — test results, medications, symptoms — please send a message through MyChart or contact our office directly.
MyChart mobile app
The MyChart app is available for iOS and Android. When prompted to choose your provider, search for and select Sharp HealthCare.
