Type 1 Diabetes Care for Adults in San Diego
Insulin dosing, pumps and CGM, hypoglycemia, and access to new technology through clinical trials.
Type 1 diabetes is a lifelong autoimmune condition, and adults with type 1 deserve care from people who manage it every day. Our endocrinologists care for adults with type 1 diabetes, whether diagnosed in childhood, newly diagnosed as an adult, or transitioning from pediatric care.
What we focus on
- Insulin dosing that fits your life: carbohydrate ratios, correction factors, exercise, travel and shift work
- Diabetes technology, including every current insulin pump and continuous glucose monitor
- Reducing hypoglycemia, including hypoglycemia unawareness, without accepting high glucose levels
- Screening for complications and for other autoimmune conditions that travel with type 1, such as thyroid disease and celiac disease
- Pregnancy planning and care around pregnancy, together with obstetric teams
- Access to clinical trials of new devices and therapies through EndoTrials
Diabetes technology
We work with all current insulin pumps and CGM systems, including automated insulin delivery (hybrid closed-loop) systems, the twiist pump, and the implantable Eversense CGM. Our La Jolla office was the first site in Southern California to insert Eversense. We help you choose a system, start it, and get the settings right over the first months, which is where most of the benefit comes from. Insulin pumps and CGM
Diagnosed as an adult?
Type 1 diabetes is often diagnosed in adulthood, and it is frequently mistaken for type 2 at first. Clues include a lean build, rapid onset, weight loss, ketones, or poor response to tablets. Testing for islet autoantibodies (GAD65, IA-2, ZnT8, insulin) and C-peptide clarifies the diagnosis. Slowly progressing autoimmune diabetes in adults, sometimes called LADA, belongs to the same spectrum. An accurate diagnosis matters because it changes treatment.
Moving from pediatric to adult care
The move from a pediatric diabetes team to adult care often happens during college or a first job, when routines change and support drops. We welcome young adults, review current settings and technology, and make the first adult visits practical rather than a lecture.
Hypoglycemia
Frequent or severe lows are a reason to see us promptly. CGM alarms, automated insulin delivery, adjustments to basal and bolus settings, and updated glucagon options can all reduce lows. People who no longer feel lows coming (hypoglycemia unawareness) can often regain warning symptoms after a period of strict hypoglycemia avoidance.
Screening relatives and delaying type 1
Type 1 diabetes can be detected years before symptoms by measuring islet autoantibodies. Close relatives of people with type 1 can be screened, and for people in the early stages, a medicine (teplizumab) can delay the onset of clinical diabetes. We discuss whether screening makes sense for your family.

Common questions
Do I still need an endocrinologist if my A1c is good?
Yes. Regular visits cover device settings, hypoglycemia, complication screening, and access to new technology and trials, not only A1c.
Can I try a new pump or CGM before committing?
Often yes. Ask us about trial periods and how to switch systems.
