Practice Policies

Cancellations and No Shows
Life happens, and we understand that. If you are unable to keep an appointment, please provide us with 48 hours notice prior to your scheduled visit. In the event of a sudden and urgent situation, please contact us as soon as possible. This will allow us to re-enter you into the schedule as well as to accommodate patients awaiting appointments or those who need to be seen due to illness or other special circumstances.

No Shows are taking time away from other patients and from our staff. Don't be a no show. If you miss an appointment without contacting us, we will charge a no-show fee of $40 - please understand that we can't reschedule you until this fee has been paid.

Medication Refill Policy

To obtain a refill for a regularly used medication, please request a prescription during your office visit. It may be necessary otherwise for you to come by our office to pick up your refill prescription. We cannot accept a fax or telephone prescription request from a pharmacy if you haven't yet established care with us or if you haven't been seen in a long period of time (usually > 12 months).

Lab Tests

In general, we do not order lab tests in advance to your first visit with us. Once you have been evaluated, our doctors will order the necessary lab tests with one of our many lab partners (LabCorp, Quest, Scripps, Sharp, WestPac, UCSD). Exceptions are handled on an indivdiual basis.

Special lab wishes can be accomodated at your own expense if in agreance with our doctors and our philosophy of practice.

After-hours Assistance
In the event of serious illness or injury occurring outside of our regular office hours, call 9-1-1 or go to the hospital emergency room. For other medical concerns needing attention prior to the next business day, call the office and provide your name and phone number. You will be contacted by Dr. Schneider or one of our staff members.

Financial Policy
We work with most major PPO insurance plans. However, questions about coverage or benefits should be directed to your insurance company or checked in your "schedule of benefits".

Please be aware that any costs not covered by insurance are due at the time of your appointment. This includes co-payment and deductible. Your insurance plan may also require you to pay a portion (“co-insurance”) of various medical care fees. For information, check your schedule of benefits or contact your insurance provider.

There is also a charge for clerical and administrative services which are not covered by insurance but may be requested by patients. Many require significant office staff time.

The fees range from $15 to $100 and include the following services:

  • FMLA forms, disability forms, insurance appeals and letters to be used for legal purposes

  • Telephone consultations: frequent or lengthy calls

​

Forms of payment accepted by our office are check, cash, insurance assignment and the following credit cards:

  • Visa

  • MasterCard

  • American Express

  • Discover


Medical Records
To obtain copies of your medical records, please call our office. We will ask you to sign a Medical Records Release Form prior to forwarding your information. If you request copies of records to be sent directly to another physician, there is no charge. However, there is a nominal service fee of $40 required in advance for obtaining copies of medical records for any other purpose.

Due to the volume of requests for medical records we require 10 business days to complete these services. Therefore please notify us well in advance of the date that you will need to obtain or forward a copy of your records.

Notice of Privacy Policy and Consent for Disclosure of PHI

Our statement of privacy policy is available in each of our offices. As a patient you will receive a copy of our privacy policy and an informed consent document for use of patient information by Dr. Einhorn / Dr. Schneider under certain legal restrictions. By law, both documents require your review and signature.
